5. Fill in your Personal and Family Details, including Name, Address, and other information, and upload the required documents.
6. Click on “Submit” to complete the form.
7. Your ABHA Card application number will be displayed on the screen, and you will receive login credentials on your registered mobile number.
8. The applications for family members will be verified by officials over the next few days.
